Unusual Locations
Industrial
accidents can occur in any location. We have investigated accidents on board
ship, in hotels, factories, schools, warehouses and fairgrounds. Reports
include comment on Statutory Duties, British Standards and Codes of Practice
where relevant. We also carry out inspections throughout the UK and Republic
of Ireland, The Channel Islands, France, Spain, Belgium etc.
Industrial Accident
This specialised piece of equipment looks secure but the work piece fell
on to the operator and investigation showed that it was unsupported. Not
protected from passing fork truck!

Proper System of Work?
The
driver was working on the cutting cylinder (which he reported was loose)
when it sprang up and injured him. He had not called for a qualified engineer
as he thought he could do the job himself.
Guards on Machines
This is a metal cross cutting saw with a defective guard in at least two
respects. It was alleged there was a complete break down in supervision,
lack of proper instruction and a breach of statutory regulations. The operator
(a young man) lost part of his forearm.
