FALLING ACCIDENTS
Falling from Height Accident
In this case, a large oil tank, approximately 14 metres x 45 metres, was
being demolished when the roof unexpectedly collapsed. We were
asked to investigate the cause and responsibility for the collapse.
The roof was supported by 21 main trusses and the collapse occurred after
four had been cut. A 'Burner' (workman) was on the roof as it collapsed
and he sustained injury. Our report detailed the correct method of work and illustrated why the structure failed.
The Flying Fox and the Open Karibiner
In this case an employee in charge of a Flying Fox event and responsible
for operational safety, fell from the ride at the end of an otherwise
uneventful day. Investigators had been unable to establish the cause.
Ted Allen studied the reports and examined the equipment. He then set
up tests and discovered that under certain circumstances, if two karibiners
rotate about each other, a karibiner with a plastic barrel can come undone.
In the court case that followed, Mr Allen's evidence was accepted as an
explanation of how the accident occurred The Judge said that he was satisfied
that Mr Allen had got it right.
